Is Intuitive Surgical Stock's Market Price Justified?

Intuitive Surgical (ISRG) stock trades at 52.4 times its earnings over the last twelve months, compared with 22.6 for the S&P 500. At that multiple, buyers are paying upfront for years of profit growth that the maker of the da Vinci surgical robot has not yet delivered. So is Intuitive Surgical growing fast enough to back a price like that?
For Now, Intuitive Surgical Grows Faster Than It Must
Partly. At this price, Intuitive Surgical would have to grow revenue about 13.6% a year for five years just to hold its market value as its P/E falls to a mature level. The company is growing faster than that today, with sales up 21.7% from a year earlier in fiscal Q3 2026, but it has to stay above the required rate in every one of the five years.
The rate relies on three Trefis assumptions. The mature P/E of 25.2 blends the stock’s own average P/E over its past twelve quarters with 22.0, the Trefis level for medical device makers. While the stock’s historical average was 66.1, the blend counts it as only 30. Trefis caps a stock’s own average at 30 whenever it is higher. The calculation also places 60% of the weight on the medical device level, meaning the mature P/E leans more heavily on that industry baseline than on the stock’s own history.
At that mature P/E, an annual net income of $6.15 billion would support Intuitive Surgical’s $155 billion market value. Reaching that profit at a steady net margin of 28.1% would require revenue of $21.9 billion. Trefis sets that margin halfway between 27.7% and 28.6%, representing the average and the peak of the company’s twelve-month net margin at each of those same quarter ends. Starting from $11.6 billion over the last twelve months, revenue reaches that target in five years by growing 13.6% a year. Trefis uses a five-year timeline because today’s P/E is 2.08 times the mature one. The rules default to five years whenever the current P/E is at least twice the mature one but less than three times, before any change for very fast or very slow growth.
Where Intuitive Surgical’s Sales Growth Comes From
Total procedures on Intuitive Surgical’s da Vinci and Ion robots grew 15% in fiscal Q3 2026, trailing the pace of sales. Management said revenue growth benefits from a richer product mix as da Vinci 5, SP and Ion become a bigger part of the business, and new da Vinci placements rose 28% from a year earlier.
Management offered no revenue forecast for 2026, providing only other measures such as da Vinci procedure growth of 14% to 15%. The company also expects its pricing actions, which include lower prices in China starting in November, to cut da Vinci instrument and accessory revenue per procedure by about 3% to 5% once fully in place, before any change in mix. Instruments and Accessories made up 59.8% of revenue in fiscal 2025, meaning that cut could narrow the gap between sales growth and procedure growth.
With Fewer Years, Intuitive Surgical Would Need Faster Growth
The biggest difference comes from the number of years. Given three years instead of five, Intuitive Surgical would have to grow revenue 23.6% a year, a pace faster than in any of its last five twelve-month periods to September. Over seven years, it would need 9.5% a year.
The company has not always grown at the pace it needs. Sales rose 11.5% in the twelve months to September 2022, and 12.0% a year later.
Profit is the other soft spot. Net margin fell to 17.1% in fiscal Q3 2026, marking its lowest point in the past five quarters. Management noted that GAAP results for the quarter reflect a pre-tax $450 million charge for an antitrust litigation settlement agreement, which remains subject to court approval. Assuming the 25.5% net margin of the last twelve months, Intuitive Surgical would have to grow revenue 15.8% a year. That twelve-month margin includes the quarter with the settlement.
For this price to hold, Intuitive Surgical has to keep sales growing about 13.6% a year as the price cuts arrive, and earn a net margin nearer its own average again. If growth slips back to the slower pace of a few years ago, the company would need more years, and the stock becomes a riskier holding. Intuitive Surgical is expected to report fourth-quarter results in January 2027, and revenue growth against procedure growth is the number to watch.
